Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.

 
General Description

The Telephone Triage Nurse will adhere to established policies, standards of care and standards of practice at Legacy Community Health Services. The RN is accountable for indirect patient care through the telephone triage system functioning as an expert communicator over the telephone providing time appropriate advice, health education and/or guidance to our clients for appointments.


Essential Functions

Accept difficult, urgent or emergency calls from patients
Using the nursing process, triages patient concerns via telephone
Assess and provide nursing advice
Gather necessary  information and/or data for provider
Documents conversation in medical record 100% of the time
Receives phone calls related to appointments, urgent care, inpatient admissions and test results in accordance with clinical protocols and coordinate proper disposition of each
Maintain an average call time of 8 minutes or less
Maintain a pleasant, helpful and professional demeanor


Education & Training Requirements

Graduate of an accredited Registered Nursing program
Current Texas licensure as an RN
Current CPR certification required

 
Work Experience Requirements

Three (3) years’ experience  as an RN, preferably in family medicine
Demonstrated good clinical, interpersonal, organizational,  written and verbal communication skills required
