    This position supports and supplements the AAS Connections Case Coordinator; Assists patients and members in receiving the optimal level of care at the most appropriate place of service; Provides support for intensive case management and care coordination services offered through AAS and the Connections Referral Program; Conducts member health screening, provides health education/coaching, and links members to appropriate community resources.


    *Retrieves, researches, and completes field referrals made to the AAS Connections Referral Program in order to link patients utilizing emergency services to appropriate community resources.
    *Assists with enrollment of patients into AAS Connections program, and supports intensive case management efforts provided by AAS Connections Case Coordinator.
    *Utilizes motivational interviewing and other techniques to engage patients and families in self-management.
    *Implements, coordinates, and monitors strategies for patient and families to improve health and social outcomes.
    *Conducts face to face home visits for non-compliant, unable to reach, and difficult to engage patients in an effort to engage the patient in their own care, and address cultural barriers when required.
    *Coordinates transitions of care by collaborating with care coordinators, insurance companies, and other providers, when indicated.
    *Facilitates communication and coordination of services between providers and clients.
    *Connects clients to community resources, including but not limited to: legal assistance, housing, food, clothing, education, life skills classes and employment.
    *Helps clients learn where to find community resources and assists with scheduling appointments with health and social services agencies and completing applications for benefits/programs for which they may be eligible.
    *Assists clients with accessing primary care and behavioral health related services, including but not limited to: obtaining a medical and/or behavioral health home, providing instruction on appropriate use of these health resources, and assisting them in overcoming barriers to obtaining needed health care and/or social services.
    *Provides support and advocacy during initial visits to health and social service agencies, or when necessary to assure client needs are understood.
    *Coordinates and monitor services, including comprehensive tracking of clients? commitment and compliance in relation to agreed upon plan of action.
    *Coaches clients in effective management of their health-related social needs.*Encourages clients to follow through with appointments, fill out applications, and assume overall responsibility for their own wellbeing.
    *Documents all encounters and contacts made on behalf of clients into appropriate database[s]. Continuously reflects back to plan of action with client to assure that they remain on track or to revise it accordingly if situation changes.
    *Acts as an advocate for the patient and family.
    *Educates patients, families, providers, and care team regarding the role of the community support worker and strategies to address health related social needs.
    *Build relationships with internal and external stakeholders including other providers, with the goal of improving the health of the community.
    *Supports AAS Mobile Integrated Health work with an interdisciplinary team.
    *Performs other functions as required.

    BA/BS in related field required, Masters degree in related field preferred

    One year of related experience required, three years preferred
